Efficacy of Systemic Glucocorticoids in Rheumatoid Arthritis

چکیده
Use of glucocorticoids (GC) began more than 50 years ago for the treatment of rheumatoid arthritis (RA). However, up until a decade ago, its potential role as a disease modifying anti-rheumatic drug (DMARD) and its capacity to prevent radiologic damage had not been proposed. Some initial studies indicated a disease modifying drug, but in the following years, probably due to questions regarding the appearance of multiple side effects, GC use started to be more cautious and few studies on their use in RA were carried out. In the past few years, different clinical trials have been published in which research into the role of low-dose GC as DMARDs in RA is carried out, as well as a characterization of side effects. They are mostly randomized clinical trials with different designs and variable followup periods; its main characteristics are summarized in Table.

Anticardiolipin Antibodies in Juvenile Rheumatoid Arthritis and Systemic Lupus Erythematosus
Background: Antiphospholipid antibody syndrome (APS) can either occur as a primary syndrome or associated with other autoimmune diseases such as systemic lupus erythematosus (SLE). Anticardiolipin antibody (aCL) of IgG and/or IgM isotype in blood, measured by a standardized ELISA is the most acceptable laboratory criteria. APS IgG isotype, particularly IgG2 subclass is more strongly associated ...
متن کاملGlucocorticoid use in rheumatoid arthritis: benefits, mechanisms, and risks.
Glucocorticoids have long been recognized to have beneficial effects in rheumatoid arthritis (RA) (1,2). Several clinical trials over the last decade have further documented the efficacy of glucocorticoids in relieving inflammation and in preventing radiographic erosions in early RA (3-5). Additionally, research has yielded new insights about the cellular mechanisms responsible for these percei...
متن کاملThe value of glucocorticoid co-therapy in different rheumatic diseases - positive and adverse effects
Glucocorticoids play a pivotal role in the management of many inflammatory rheumatic diseases. The therapeutic effects range from pain relief in arthritides, to disease-modifying effects in early rheumatoid arthritis, and to strong immunosuppressive actions in vasculitides and systemic lupus erythematosus. There are multiple indications that adverse effects are more frequent with the longer use...

All patients were admitted for clinical and serological evaluation. In terms of sex ratio, pattern of joint involvement, severity and nodularity, RA in east Iran did not resemble that which has b...
متن کاملSulfasalazine plus Chloroquine-Induced Mood Disorder in a Patient with Rheumatoid Arthritis
Rheumatoid arthritis is a chronic systemic inflammatory disease that affects approximately 0.5-1% of the world population. The current approach to this disease is to start an intensive treatment without delay once the disease has developed. Various studies in the literature have shown that combination of disease modifying antirheumatic drugs such as sulfasalazine and chloroquine offers a more a...
